Dentists are calling for a permanent change in the way overlimits are settled. It’s about payment terms.

The dental community is pushing for a change in the accounting treatment for dental services provided by the National Health Fund. The current system does not meet the expectations of doctors. They are forced to beg for their own money and often remain unpaid for several months.

Money for treatment is paid on time, only within a certain limit. If the dentist decides to accept more patients, he has to wait for payments, even several months. Benefits that exceed the established “norm” are calculated by the National Health Fund quarterly from the so-called Medical Fund (money for the first quarter ended in March went to dentists on May 8, 2023). In practice, this means that the owners of dental offices and clinics often finance the treatment of patients admitted to the NHF from their own funds.

Dentists are outraged by the payment deadline

“WHO […] would agree to defer remuneration for several months, and we are not allowed to lower the standards because it would pose a threat to patients and legal consequences for us,” says Dr. Magdalena Shevchik. Doctors emphasize that the current way of financing additional dental services is unacceptable. Michal Bulsa, chairman of the District Medical Council in Szczecin, describes this mechanism as a kind of “pyramid scheme” where the payer - in this case the National Health Fund - incurs debts to entrepreneurs (owners of dental offices), promising them a delayed refund.

The debt for payment of money for overfulfillment in some cases reaches PLN 160,000. “My girlfriend is left without a salary for several months, and only when the overpaid money comes in does she receive her salary. It shouldn’t look like this,” adds Dr. Magdalena Shevchik.

National Health Fund sends protocols to doctors

Dentists have not yet received an official response from the National Health Fund in this case. However, the National Health Fund has already sent protocols to some doctors for exceeding the limits. Still no money. What is important, as Dr. Shevchik points out, is the need for ongoing changes in how dental services are funded, not a one-time “risk.”
